All services provided to you, including any psychiatric treatment, drug and alcohol treatment, as well as abuse or HIV status, are documented in hospital records, which are hospital property. Every time we need to use or disclose your records, we must provide you with notice of our legal duties and privacy practices. When your protected health information is used or disclosed, we are required to abide by the terms of this notice (or other notice in effect at the time of the use or disclosure). Upon request, you may obtain a paper copy of our Notice of Privacy Practices, which goes into detail about how we use or disclose your information.
If you desire further information about your privacy rights, or if you are concerned that your privacy has been violated, or if you disagree with a decision that we made about access to your protected health information, you may contact our Chief Privacy Officer. You may also file written complaints with the Director, Office of Civil Rights of the U.S. Department of Health and Human Services. Upon request, the Privacy Officer will provide you with the correct address for the Director. We will not retaliate against you if you file a complaint.
